- Consent to Participate in Skills Training Programs
I, the undersigned parent/legal guardian of the participating student, hereby
agree to the following: - I understand the nature of the training and workshops provided by Skills
Connect Academy. - I acknowledge that all activities are non-hazardous and educational in
nature. - I consent to allow my child to participate in these programs, whether
in-person, at-home, or virtually. - I accept full responsibility for ensuring a safe learning environment when
sessions are conducted in our home. - I agree that my child will follow class expectations and be respectful toward
staff and fellow students. - I will notify the program in writing of any allergies, learning accommodations,
or medical needs relevant to participation.
This consent remains valid for all enrolled programs unless revoked in writing.
